Everybody Sells Reminder Texts. Almost Nobody Cites a Study.

The "40% fewer no-shows" number in booking-app marketing has nothing under it. The real research runs to tens of thousands of people, says something more modest, and is far more useful.

Search for whether appointment reminders reduce no-shows and you will meet the same number again and again: reminders cut no-shows by 40%. Sometimes 38%. Sometimes "up to 80%." Follow it back to where it came from and you land on a booking company's blog, citing a second booking company's blog, citing nobody at all.

We went looking for the bottom of that pile. There isn't one. The number is furniture — everybody repeats it because everybody repeats it.

What does exist, once you dig past the marketing, is a genuine pile of research. It's all from medicine, not barbering. The effects are smaller than the claims. And it's a lot more useful than a number somebody made up.

Reminders work. Not by 40%.

The best single source is a Cochrane review — the boring, thorough kind that pools every trial it can find and refuses to round up. It gathered eight randomized trials, 6,615 people, and compared text reminders against nothing at all.

Read that as a percentage of appointments and a text buys you eleven points. Read it as a percentage of your no-shows — which is the number that actually costs you money — and it is better than it looks:

A third. Not 40%, not 80%, and not from a company trying to sell you anything. That is the honest ceiling on the single cheapest thing you can do about no-shows.

Two more things from that review are worth knowing. Text reminders performed about as well as staff ringing people up — statistically the two were a coin flip. And per appointment actually attended, texts cost between 55% and 65% of what the phone calls cost. If you have ever spent an evening calling tomorrow's clients one at a time, that is the trial telling you to stop.

One text, or two?

This is where the vendor blogs get loudest and where, until recently, nobody had really checked. Then a three-armed randomized trial ran across 25 clinics and 54,066 people, testing exactly the thing every booking app has an opinion about: is one reminder enough?

Two reminders won. Against the better of the two single-reminder schedules it removed roughly a sixth of what was left:

Among the people the researchers had flagged in advance as most likely to miss — the ones with a history of it — the same pattern held and mattered more: 20.5% missed with two reminders against 24.2% with one.

So the answer to "one or two" is two. Not because it doubles anything. Because the second one is close to free and it keeps working on precisely the people who were going to ghost you.

"Won't two texts annoy them?"

Fair objection. Nobody wants to be the business that pesters.

The trial measured it. Visit satisfaction was unaffected by the extra reminder — the second text did not cost them anything in how people felt about the appointment.

Which tracks, if you think about what the message actually is. A reminder isn't a demand. It's not "per my policy." It's a note that says hey, the thing you agreed to is happening tomorrow — and here's the easy out if it's not going to work. Most people are quietly relieved to get it.

What a reminder cannot fix

Here is the part the 40% number is designed to skip.

A separate review of 26 studies looked at why people miss appointments in the first place. Forgetting was one of the most commonly given reasons — but it shared the top of the list with work and childcare commitments, and with transport. Being too unwell to come showed up too. So did the weather.

A text fixes exactly one of those. It does nothing about a shift that got moved, a car that would not start, or a kid sent home from school. That same review found missed appointments averaged 15.2% across the studies that measured them, with a spread from 3.3% all the way to 48.1% — which should tell you how much this varies by who your clients are, and how little a single industry-wide number is worth.

Reminders are the cheapest slice of the problem, and they are the slice you can automate completely. They are not the whole problem.

Every number on this page comes from healthcare. Clinics, GP surgeries, hospital outpatients. We could not find a single randomized trial of appointment reminders run in a barbershop, a salon, or a tattoo studio — only companies selling reminder software, quoting each other.

Some of it should carry across. The mechanism is memory, and memory does not know whether the appointment is a blood test or a beard trim. Some of it should not: your clients chose to come and are paying for something they want, which is a different situation to a follow-up somebody is dreading.

Even the Cochrane team admits their own evidence isn't strong enough to build policy on. If they won't oversell eight trials, a booking app definitely shouldn't oversell zero.

Here's what we'd take from it: the direction is solid, and it costs basically nothing. That's enough to act on. It's not enough to promise you a number — and anybody who does is quoting a blog post, not a study.

What to actually do

The research supports something unglamorous:

  • Send more than one. Two beat one in the only large trial that tested it properly. One is most of the benefit; two is the rest of it.
  • Space them out. One far enough ahead that cancelling is still useful to you — you want the slot back with time to refill it — and one close enough to the appointment to catch the person who genuinely forgot.
  • Make cancelling easy. A reminder that only nags is worse than one that offers a way out. A cancellation two days early is a slot you can sell. A no-show is not.
  • Send them automatically. The trials tested automated messages. A system that depends on you remembering, at the end of a nine-hour day, is not the thing that was tested — and it is the thing that quietly stops happening in week three.
